Please park in the back parking lot and enter through the front of the building. Come up the stairs to my office on your right. If the door is locked I am in session; please do not knock. The waiting room is located in the second story enclosed porch area. Make yourself at home and I will be with you shortly.

Acupuncture registration must be completed by the start time of your first appointment by any of the following ways: print BOTH links found in the 'Registration' tab above to complete, request a hard copy in the mail, or arrive 15 minutes early to your scheduled exam time. 

Biofield tuning registration has only one form- that can be completed quickly upon arrival. 

All registration forms are available online (see registration page here) and in a hanging file folder to the right of my office door.
